I-824: Application for Action on an Approved Application/Petition

Complete and file Form I-824 with the USCIS to request:

  • a duplicate notice of approval of an approved petition or application;
  • that another consulate be notified of the approval of the petition; or
  • that a U.S. consulate be notified of your adjustment to permanent resident status so your spouse/children can apply for immigrant visas.

TIP: Include a copy of the original notice of approval, if available, to speed things up.

I-134: Affadavit of Support

Use Form I-134 to provide evidence of financial responsibility for the visa applicant in those instances when form I-864 is not used.

G-28: Notice of Appearance

Use Form G-28 to notify USCIS that the alien has authorized you to represent him/her in the matter pending before USCIS. This Form must be completed by the representative and signed by both the representative and the alien. Form G-28 usually accompanies the aliens initial filing of a petition or application, but can be submitted to USCIS at a later date if the representative is engaged after the date of filing. USCIS restricts who may represent an alien and the terms of engagement.

G-639: Freedom of Information Act/Privacy Act

Use Form G-639 to obtain records from government files relative to you or someone elses immigration matter.

IRS-9003: Additional Questions To Be Completed By All Applicants For Permanent Residence In The U.S.

Complete and attach Form 9003 in duplicate to Form I-485 when applying for adjustment of status.
