Use Form N-565 to apply for a new:

• certificate of citizenship
  
• certificate of naturalization
  
• certificate of repatriation
  
• declaration of intention; or
  
• special certificate of naturalization to obtain recognition of your U.S. citizenship by a foreign country.

Typically, you will apply for a new certificate if yours was destroyed, mutilated, lost, contains an error, or if your name has changed.

Use Form N-600 to obtain a certificate of citizenship if you were born outside of the U.S. to parents one or both of whom was/were an U.S. citizen(s) at the time of your birth, or whose parents have become naturalized citizens. You must be physically present in the U.S. to file this Form.

Form N-643 is used to obtain a certificate of citizenship for an adopted child of U.S. citizen parent(s).
