I-102: Application for Replacement of Arrival/Departure Document

Use Form I-102 if you are a nonimmigrant and wish to replace a lost, mutilated, or destroyed Arrival/Departure document (I-94, I-94W, I-95).

I-131: Application for Travel Document

Complete Form I-131 if you intend to depart the U.S. temporarily and one of the following applies to you:

  • You are a permanent resident physically present in the U.S.,and will be departing the U.S. for a continuous period of 12 months or longer. You wish to preserve your legal permanent resident status. File Form I-131 and request a Re-entry permit;
  • You are physically present in the U.S., awaiting adjustment of status to permanent resident (i.e., you have already filed I-485 but, to date, INS has not stamped your passport with a rubber stamp that reads I-551). File Form I-131 and request Advance Parole. NOTE: Do not leave the country on advance parole or otherwise if you are subject to the 3/10 year bars to readmission.
  • You are physically present in the U.S. as a refugee (before and after adjusting status to permanent resident) and will depart the U.S. temporarily. File Form I-131 and request a Refugee Travel Document; or
  • You are applying for someone else who is outside the U.S. and eligible for advance parole under certain specific circumstances.

N-470: Application to Preserve Permanent Resident Status

Use Form N-470 to preserve your status as a permanent resident for naturalization if you're leaving the U.S. for a period of 6 months or more and you're employed by:

  • the U.S. government;
  • a U.S. research institute;
  • a U.S. corporation or majority-owned subsidiary of a U.S. corporation engaged in foreign trade or commerce; or
  • an international organization which employed you after you became a permanent resident and of which the U.S. is a member.

I-90: Application for Replacement of Alien Registration Card

Use Form I-90 if you are a legal permanent resident of the U.S. and wish to replace an obsolete, mutilated or lost green card (I-551), or to change a name on your green card.
